Passport Services

New Passports

If…

  • you have never had a passport
  • your last passport was issued more than 15 years ago
  • you have lost your previous passport
  • your previous passport was issued when you were under the age of 16
  • your previous passport was damaged or mutilated beyond normal wear and tear
  • your previous passport was limited from the normal ten year validity period due to damage/mutilation or multiple passport losses/thefts or non-compliance with 22 C.F.R. 51.41 (please refer to the back pages of your U.S. passport book for endorsement information)
  • you will NOT use the same name as your previous passport or you do NOT have certified documentation to reflect a name change (ie. marriage or court order)

…we are happy to execute your passport application for you. We have copies of all necessary forms available for pickup and our passport service is by appointment during the following hours:

Passport Renewals

If…

  • your passport was issued less than 15 years ago
  • you were over the age of 16 when it was issued
  • your previous passport is in your possession and was not damaged or mutilated beyond normal wear and tear
  • your previous passport was NOT limited from the normal ten year validity period due to passport damage/mutilation, multiple passport thefts/losses, or non-compliance with 22 C.F.R. 51.41 (please refer to the back pages of your U.S. passport book for endorsement information)
  • you will use the same name as on your most recent U.S. passport or you can submit certified documentation to reflect name change by marriage or court order

…you are most likely eligible to renew your passport using the DS-82 renewal form. This is a self-service form that DOES NOT need to be executed by a Passport Acceptance Agent. We are happy to answer any questions you may have about the renewal process and to take your new passport photo, but please be aware the Library CANNOT handle your renewal. Photo Services

Passport photos have specific requirements. Please visit Passport Photos for more information regarding these requirements. We can take your passport photos at the library for $10 per person.

What You Need to Apply for a Passport:

  1. Form DS-11
    • If you fill it out ahead of time, please fill it out in black ink or use the form filler on the State Department website.
    • Please do not cross out or use white-out if you make a mistake.
    • DO NOT sign the application until instructed to do so by an Acceptance Agent.
  2. Original Proof of US Citizenship and Photocopy:
    • Original or Certified Copy of Birth Certificate (if born in the United States)
    • Original Naturalization Certificate
    • Previous US Passport
    • Original Certificate of Citizenship
    • Consular Record of Birth Abroad

  3. Proof of Identity and Photocopy:
    • Valid New York State Driver’s License (please photocopy front and back)
    • Passport
    • If your ID is from another state, please bring a secondary photo ID with you. For acceptable forms of secondary ID, please visit Photo Identification
  4. Photo:
    • Every applicant needs to submit a 2” x 2” photo taken within the last six months. This can be taken at the Library for $10 per person.
passport fees

Fees (for routine processing):

  • Adult Passport Book: $130
  • Minor Passport Book: $100
  • Adult Passport Card: $30
  • Minor Passport Card: $15
  • Adult Book and Card: $160
  • Minor Book and Card: $115

For the purposes of Passport Services, 16 and 17 year old applicants are treated as adults and pay the adult fees.

These fees MUST be paid by check or money order payable to the US Department of State, and should be separate for each application.

The Library is required to charge a $35 Acceptance Fee per application. This fee can be check, money order or cash.

If you need your passport more quickly, you may pay a $60 per application expedite fee. This fee is payable by check or money order to the US Department of State.

You may have the Library overnight your passport application. The charge for this service is $28.75, payable to the Library.

You may also have your passport shipped to you overnight. The charge for this service is $18.32, payable by check or money order to the US Department of State.

passports for minors

For Minors (under age 16):

Minors must be present at their passport appointment with BOTH of their parents. If only one parent can attend, the non-attending parent must sign and have Form DS-3053 notarized, as well as provide a photocopy of the front and back of their ID.

Minors MUST provide their birth certificate even if they have had a previous passport. When applying on behalf of a minor, both parents MUST submit photocopies of their IDs.

For Applicants Ages 16-17

Applicants who are 16 or 17 years old should have one parent present at the appointment. It is recommended that applicants in this age bracket provide their own ID, however, if they do not have their own ID, one parent may provide their ID instead. Please visit Applicants Ages 16 and 17 for more information.

Everyone’s situation is different because of name changes, adoption, custody arrangements or other extenuating circumstances.
